Horizon Actuarial Settlement Payout
While agreeing to a $8,733,446.36 payment to end the data breach class action lawsuit, Horizon has not acknowledged any wrongdoing.
Class members who suffered unpaid damages as a consequence of the data breach are eligible to receive up to $5,000 in payment for out-of-pocket costs under the terms of the settlement.
To be eligible for this compensation, applicants need to provide proof of the damages they experienced. Additionally, for five hours of missed work, class participants may be reimbursed up to $125 at a rate of $25 per hour.
- Up to $5,000 in reimbursement for real, verified, and unreimbursed out-of-pocket expenses stemming from the data security incident.
- Up to five hours at a rate of $25.00 per hour, or a maximum of $125, will be paid for time spent resolving problems associated with the data security incident.
- A maximum cash payout of $50 will be given to each Settlement Class Member who makes a claim, and an extra $50 will be given to individuals who were residing during the Data Security Incident in California.
Every claimant is eligible for a one-time payment of about $50. Californian claimants are eligible to earn an extra $50 in compensation.
How to get the Horizon Settlement Payment?
To receive payment from the Horizon settlement, you must do the following actions:

- Wait for court approval and processing: On March 25, 2024, the Court will convene a Final Approval Hearing to determine whether to approve the Settlement. If accepted, there may be appeals, and they might take weeks to process. Additionally, processing every claim form takes time. It’s advisable to be patient throughout this time.

If Settlement Class Members were notified directly, they must provide their Unique Class Member ID number; if they were notified by publication, they must certify under penalty of perjury that this Data Security Incident affected their personal information and provide their name and address.
- California Residents: Settlement Class Members may be eligible for an additional payment of up to $50 if they were residents of California at the time of the data security incident.
